Scheduling Your First Visit. 🗓️
Prefer that we call you? Email us your name, insurance information, and phone number, and we'll reach out to schedule your appointment. Please note: this initial contact is for scheduling only, so do not email medical documents or records.You may also call our office directly, and we'll do our best to find a time that works for you.If you haven't had an annual physical recently, we recommend beginning your care with us by scheduling lab work and a comprehensive physical exam. This visit forms the foundation of your care — it allows your physician to build a complete picture of your health and create a sensible plan going forward.
What to Bring to Your First Visit
- Filled out New Patient Form
- All medications you're currently taking — bring the actual bottles or a photo of each one, including prescriptions and supplements
- Recent lab results (if available)
- Relevant medical records, especially recent MRI, CT, X-ray, or colonoscopy reports
- A list of your current and past medical conditions
- Home blood pressure readings, if you track them
- Your immunization records
- Your insurance card and driver's license
Please be prepared to cover any co-pays or deductibles, and plan to arrive at least 15 minutes before your appointment.
Forms and Medical Records 📄
- New Patient Forms: Complete your clinical history forms before your appointment, then fax them to 972-981-3600 or bring them with you. If you have trouble completing them, we'll gladly help you during your first visit.
- Records Request Form: Download the form needed to request your medical records from a previous provider.
Tests, Screenings, and Prevention
Our office can obtain or arrange any blood or urine tests you may need, and our staff can schedule X-rays and ultrasound studies at your convenience. Please be aware that some screening studies may not be covered by your insurance; we'll let you know in advance which ones are unlikely to be covered. Regular exams and screening tests are among the most powerful tools in medicine: detecting problems early greatly improves the chances of effective treatment. What you need depends on your age, health, family history, and lifestyle — so don't hesitate to ask your doctor whether it's time for a particular screening or vaccination. Some tests have specific preparation requirements; knowing these ahead of time helps you get the most out of every visit.While no exam can guarantee protection from every illness, investing in preventive care and continuity of care today can meaningfully improve your quality of life in the years ahead.
Getting the Most From Your Care
Set up your patient portal. At your first visit, we'll register you on our patient portal, where you can update your information, pay bills, view lab results, request prescription refills, and message your provider.
Keep a running list. Note any changes in your health or habits — when they started, how they differ from before, and anything else you think might be relevant — and bring it to your visit. It's also helpful to jot down your provider's recommendations.
Be open with us. Your care plan is built on the information you share. The more accurate and up to date it is, the better the guidance we can give you.
We're a team. If your doctor is away, our mid-level providers and our Plano office are available to care for you.
