Alternatively you can also call us and we will do our best to help you schedule a visit.
We believe you deserve to feel as welcome and comfortable as possible when you’re sick, injured, or simply maintaining your health. We believe you deserve high-quality care and consultation, no matter what brings you in. And we believe you deserve respect — for your time, your schedule and your vision for your own health.
We’re not just concerned with what brings you here today, we also want to understand if that might be connected to something that could impact you in the future. It means taking a bit more time to let your doctor connect the dots that form the larger picture of your health. Trusting us to keep track of that larger picture means you can focus on getting and living well today, as well as in the years ahead.
We are happy that you have chosen us as your primary care doctor. We suggest that if you have not had an annual physical exam you start your relationship with us by scheduling labs and a physical exam. The basis of your medical care rests on a comprehensive yearly physical examination. This allows your physician to establish a complete database on your health and to create a sensible plan for your future.
We ask that for your first visit you bring a copy of your last labs (if possible) and any pertinent records (particularly a copy of the report on any recent MRI's, CT's, x-rays, colonoscopies, etc...). This will help us get to know you better and serve you better.
If possible bring all of your medicine bottles or take a photo of each bottle with your phone and bring that photo. Do bring with you any home blood pressure readings. Do also bring a copy of any immunization records.
Any additional blood or urine tests that you need can be obtained or arranged by our office. X-rays and ultrasound studies can be scheduled by our staff at your convenience. Please note that all studies recommended for screening purposes may not be covered by your insurance. We will let you know which studies are likely not to be covered. You should also be aware that medical examinations are never foolproof and cannot ever be guaranteed to protect you from any and all illnesses. However, the investment you make in your health today with this type of evaluation and continuity of care is much more likely to add years of quality time to your life than the alternative.
Please bring a copy of your insurance card and be prepared to pay for any co-pay or deductibles that your insurance requires. Please arrive at least 15 minutes prior to your appointment.
Our goal is for you to feel comfortable and well cared for. At your first visit we will set you up for our Patient Portal. Once you are on our portal you can update demographics, pay bills, view lab results, request refills and send a message to your provider via the patient portal.
We work in teams and if for some reason your doctor is on vacation or not available we do have mid level providers and a Plano office that can also help with your needs.

Regular health exams and tests can help find problems before they start. They also can help find problems early, when your chances for treatment and cure are better. By getting the right health services, screenings, and treatments, you are taking steps that help your chances for living a longer, healthier life. Your age, health and family history, lifestyle choices (i.e. what you eat, how active you are, whether you smoke), and other important factors impact what and how often you need healthcare.
The need for certain health screenings, tests, and vaccinations can be based on your age, general health, family history, and lifestyle. Ask your doctor if it’s time for any vaccinations or screenings. Some tests have certain requirements in order to be effective. Having this information ahead of time can help reduce your number of trips to the doctor’s office and make sure you get the most from each visit.
Write down a list of any changes in your health or habits so you don’t forget them. Note how it’s different from before, when the change occurred, and any other observations that you think might be helpful. Also, take notes on what your provider recommends.
